Default New yorker assistance required please

1993 new yorker fifth ave v6 starts but dies immediatly. If i feather the throttle I can get it to run but run badly flash codes of 12 and 21. Sons car his words were at a friends started car died started again and ran fine then otw home started running rough and said he had to keep his foot on the pedal to keep it running. First issue i saw was it was very low on gas. put 5 gallons in it cycled the key 3-4 times same issue checked the fuel shradder had fuel there. The only work done to this car was the solonoid pack and speed sensor on trans. Also listened and the fuel pump appears to be working can hear it run when the key is turned to on. Hope someone can lead me in a good direction thanks
