Exclamation Fuel Rail Pressure Monitoring HELP PLZ ????

Hi guys i have a 2003 Voyager 16000,000 miles on the clock is a 2.5 CRD so fear i have changed the injector 1 and the senor on the fuel rail pressure. but the car keeps losing power on me and it is showing up on the ECU its the fuel Rail Pressure monitoring now i have no idea where to start looking for this as i have 5 young kids traveling in the car every day on the Courtney Roads

Default

mprop valve on the back of the fuel pump can be a common issue also change fuel filter ,and maybe drop tank to check the gauze filter on the pick up
