uk 2.8crd voyager

hi and hello to all
need hellp or advice i have engine oil in water expation bottle filler
black engine oil is getting into the water i think via the oil water cooler

please any picture or manuals of 2006 diesel 2.8crd engine showing oil water cooler
and any advice or help on how to change,,, fault find
