2005 Pacifica Wheel shake
 

Have a slight steering wheel shake...thought it was rotors/brakes. Replace both, actually all 4, still there.

Jacked up the car again, if I hold the driver side tire and 9 and 3, and try shaking the wheel, it moves a slight bit. If I hold at 12 and 6, does not move.

Tried the same with the passenger side, does not move at all for either.

Guessing this is the problem, but what is the issue? Ball joint? Bearing?

Could be balance.
 

Have you had the tires balanced lately. I had the same problem and it ended up being that I had lost a weight on one of the front wheels. Balancing fixed the problem.

Balance was done as well...

I think me being able to wiggle the driver side as described is an issue.

Check the tie-rod end.

I agree with the previous poster. 9 and 3 is typically tie rod end. Outer more so than inner in my experience. Hold onto the tie rod end where it meets the knuckle while you or someone else moves it and you can usually feel it.

Is that an easy fix? Should I be concerned driving it like it is?

It's an easy fix. You'll need something to separate the ball joint stud, either a pickle fork or a puller. Many parts stores will rent or loan the tool. Count the turns when you thread off the end from the tie-rod so you can thread on the new onethe same number of turns and preserve your toe-in adjustment. Or pay for a wheel alignment afterwards. Probably the best plan.
If you wait until it fails completely you can lose steering. Not a good development.
