Default EGR Valve, EVAP?

The check engine light came on and it coded to gas cap-regular occurance. New cap in, but light came back on coding to EGR Valve. I replaced the EGR valve, but I am having great difficulty figuring our where all the hoses go to. Don't know where the hose that attaches to the underside of the plastic valve piece. The is a hose with a yellow 90 degree fitting on it-same thing. There is a fitting on the top of the throttle body that is supposed to be for EVAP, but I don't know what that is supposed to connect to. Sorry for not being up to speed on what I took apart a few days ago. I had a lot of trouble getting the throttle body mount back in to place and I guess I misplaced the hoses. REALLY appreciate the help. We love out PT. 156k miles and still going strong.
