Default 2013 Town and Country 480 code

I have read several threads on 480 code but am not sure where components are on a 2013.

The code set almost all times after I clear it. A few times it goes a couple of days before setting.

After warming up the temperature reads 192 to 194 F
I hear the slow speed and high speed radiator fans running at times when outside by radiator.

I know and have checked the resistor and fuse.
Is the relay behind headlamp as some have said on some models? I stopped at dealer and parts and service would not answer any questions. Are there any diagrams that show where components used in fan control circuit

Found and checked relays and they work good when power and ground are added to terminals.
