thud sound when braking

Default thud sound when braking

own a 2001 T&C. Have a thud, thud, thud sound from the left front when braking. When braking very lightly, no thud sound. Press the brake a little harder have the thud sound. Press a little harder and no sound. Only does it at slow speeds (below 5 mph). Sometime does it when making a turn. Does NOT do it in reverse. I'm thinking rotor or pads. Any ideas?

Ok found out it was loose lug nuts. Now my question is why would they come loose? When i torque them after a tire rotation, suspension work, etc. i always torque at the mfg. specs plus 5 ft lbs., so why would they come loose.
