Default Car Jerking

Hi there,
I'm using a Chrysler Grand Voyager 3.3L,yr 2000.
Today,while driving fir a short distance around 5km,suddenly the car starts to jerk slowly. I could feel it was rough.
After awhile,the jerk went off. I then park the car.
Then when I start to drive back,the jerk was gone but came back after a short distance n the ENGINE LIGHT appears. As for the gear change,it was normal but the jerk continues untill I reached home.
Could it be the distribution box,if it is,I've change it early last year.
Please assist me,tqvm.

The engine light indicates a fault code is stored. Have the codes read with a scanner. The code will narrow down the dozens of possible causes.
