Default 98 Sebring Shift Issues

Well that has been going on for a while and now especially that it is getting colder. When I start my car and take off the car goes and then it will shift down or into neutral, then catch then drop into neutral then catch then drop then catch around 10-12 times then finally holds. Each time it drops the car will jump as high as 6k rpms. Then after that fit, if I have to stop or slow down, when the car shifts from 2nd gear to 3rd gear it drops and the motor revs up to 4-5k then jumps when it catches and im good to go again. After about 30 mins the car will do its shift jump and then get stuck in 1st gear and im driving at 45mph at 4k rpms. All i have to do is stop, turn off the car and turn it back on and I am good to go again. This leads me to believe it is not mechanical otherwise restarting the car would not "fix" it. From everything I have been able to gather it sounds like the Transmission Control module or transmission control sensor, is this correct? What is the exact name for the module and/or part number? Thanks a lot!!
