Drum to disc change. Possible?

Default Drum to disc change. Possible?

I have a '98 3.3 Grand Voyager. Excellent car, apart from (in my opinion) the rear drum brakes. I have been toying with the idea of swapping the complete rear axle with a disc brake model.
Does anyone know if this is possible and straightforward?
Do the hydraulic lines fit into the calipers without modification, and will the drum brake parking brake cables fit straight on to the disc brake axle?
All advice greatly received. Thanks.
