Serpetine belt 2000 T&C

I own a T&C I think it is a 2000 and I had my my serpetine belt replaced a few months ago. A few weeks when I drove through a puddle the belt came off and luckely I was able to get the belt back on, I thought well the belt must have came off its tensioner when it got wet from driving through the puddle. Well my wife had the same thing happening yesterday. I am pretty sure that the belt was replaced with adn OEM belt, my question is do you think the tensioner needs to be replaced also since this has now happened twice in two months and never before that?

There is an updated part to fix this problem. I don't have the TSB number off the top of my head but I believe you replace the idler pulley and it should keep this problem from coming back. Just be sure to avoid the big puddles.
