Default 5mt diagnosis help?

2004 PT Cruiser, Manual Transmission (A-578)
transmission is also used in similar age Dodge Neon

Looking for help diagnosing rattle from manual transmission. Started as randomly popping out of third gear and sometimes reverse, then on one startup began making the noise heard in this video. Noise is not present when clutch is engaged, but is present even in neutral when clutch is released. Actions taken:

- replaced clutch pack and throwout bearing
- replaced input shaft bearing
- inspected all gears and shafts for damage
- thoroughly cleaned interior of transmission
- pre-lubed bearings with ATF-4 (specified fluid)
- as specified, refilled with 2.7qt to within 3/16" of fill hole while level

Despite removing, performing the above, and reinstalling transmission TWICE(!) the noise remains and the performance issues (popping out of gear in 3rd and reverse) continue.

Noise is concerning enough to not want to drive the car until either determined to be repairable or to understand that it is on its last legs. Hoping someone with manual transmission knowledge can view the video, hear the sound, and lend some advice. Thanks!

1) you need motor mounts, desperately.
2) that sounds like a throw out bearing rattle.

the throw out bearing is held on to the fork by really small and fragile tabs. they like to break. ask me how I know. you'll get that rattle periodically if the tabs are broke and the bearing rattles around a little.
the other option is, the bearing is failing, but you've replaced it you said. the fact it goes away when you press on the clutch leads me to believe it's the former, not the latter.

as for the popping out issues, that's an internal problem. I can't find any data on if the A-578/T-350 use the same shift selector as the T-850 (which I'm more well versed in) so I can't tell you if the extended versions would help that.
