Default '02 T&C Spitting coolant

'02 T&C Limited with 307k miles. She has been getting hot and losing coolant from what looks like somewhere around the radiator cap. Its spraying onto the belts as well. She can sit in the driveway running and it won't get hot, but under load such as driving for a longer distance (>5 miles)when the hood is opened there is coolant all around the radiator cap and under the hood in that area.
Replaced the radiator a year ago due to a leak. Top radiator hose was replaced in November d/t blow out. Have replaced the radiator cap twice and the thermostat once. She has been to the garage and we were told she holds pressure so it has to be the radiator cap and they changed it for the 3rd time. When we got home after picking her up there was coolant. Called the garage and told them they were wrong, we were told she's got high miles and should consider due to her age replacing her. There has to be a trackable and replaceable problem. Ideas? Please?

put a pressure tester on it see if it leaks

Yes sounds like head join blow-by. Get garage to check for fumes in the radiator tank.

The pressure test was completed at the garage. It held pressure with no leaks.

Gas analyser for bubbles in coolant. The other answer is the coolant pump. Some cars are known to loose the impellor on the shaft (comes off). Others have plastic ones that crack. Don't know if this is a known problem on your engine.

Default New finding

Checked for blow-by with the pinching the PCV tube and watch the oil cap trick. There is actually suction coming from oil filler cap.
Of course whenever the radiator cap is opened after driving it spews coolant even if she is cold. (built up pressure from previous drive?) Today after adding coolant, started her up with radiator cap on, found that upper hose was suctioned flat. Opened cap to release suction. What would cause that? Is it possibly related to the same problem?
