There should be a resistor in the blower area that has probably gone bad. It seems to be a common problem.
Less commonly the control switch itself can go, or part of your wiring harness has come loose.

It's a blown resistor. The fact that it only works on "4" is because the circuit bypasses all resistors on 4. Use the search function on site here and you'll find lots of discussion on the issue including step-by-step replacement instructions. It's an easy do-it-yourself repair. Only caveat is if you are a really big person you may have difficulty bending down under the dash.
