Default Erratic tickover

My 2007 2.8 diesel Voyager has developed an erratic tick over, occasionally.

It only seems to be when it's warm after a motorway run.
It's ok when it's cold or on short runs.

When it is rough it also seems to have a noise very much like a tappit sound but it's not there when the tick over is smooth.

The other thing that seems strange is that when in park it will only rev to 2.5k. Is this normal or is it in some sort of limp mode, it revs well over that and drives fine.

The only recent work was a replacement alternator but I can't see a connection with that.

Any ideas or suggestions would be appreciated.

Thanks

Picked this out of another thread regarding this sort of noise.

Here's an interesting answer:- Slapping at Idle. Loudest at Idle. Calms on Acceleration. No smoke. No loss of power.
Its called Injector knock. Caused by a faulty sticking injector..Only happens at idle but ok all other times. Lazy injector has been mentioned.
